Join us on December 3, 2024, from 2:00 PM – 3:30 PM ET, for a Milbank State Leadership Network briefing held in collaboration with IMPaCT Care. IMPaCT Care will highlight its forthcoming toolkit, which includes model State Plan Amendment text, the rationale behind the model text, and principles for CHW/P/CHR reimbursement rates. State executives from New Mexico and South Dakota will provide insights on their own journeys to implement CHW/P/CHR payment policies.
