Aging – Rebalancing Long-Term Services and Supports and the Integration of Medicare and Medicaid Services

In 2050, the population aged 65 and over is projected to be 83.7 million, almost double the number it was in 2012. The needs of this aging population will have wide-ranging implications for the country, presenting both challenges and opportunities for families, businesses, health care providers, and public programs.  As the landscape of elder care continues to evolve, states need to develop policies and programs that can navigate the challenges of complex care management needs, patients eligible for both Medicaid and Medicare (dually eligible), workforce shortages, and more.

In the fall of 2016, state policymakers participating at the Reforming States Group (RSG) meetings discussed the barriers they face in rebalancing long-term services and supports and the integration of Medicare and Medicaid services to reduce the gaps, duplication, and fragmentation of care. National experts and state leaders presented the hallmarks of integrated Medicare-Medicaid programs and how states can rebalance institutional and home- and community-based care while maintaining or reducing institutionally based care.
