This month, the Milbank Memorial Fund joined a collaborative of five foundations working together to improve the health outcomes and daily lives of individuals with complex care needs. This group of prominent philanthropic organizations, which includes The Commonwealth Fund, The John A. Hartford Foundation, the Peterson Center on Healthcare, the Robert Wood Johnson Foundation, and The SCAN Foundation, aims to accelerate health system transformation through their combined efforts.
One step toward achieving these aims is the Playbook, funded by the original five foundations, and developed by the Institute for Healthcare Improvement. It provides users with the best available knowledge about proven or promising approaches and encourages them to test best practices in their own care settings. The Milbank Memorial Fund will contribute to the group by enhancing the now six foundations’ collaborative connections with multi-payer primary care transformation implementers at the regional and state levels.
