What fosters sustainable primary care transformation? What stands in the way? These questions are the focus of a Rocky Mountain Health Plans’ Issue Brief that highlights the work of the Milbank Memorial Fund’s Multi-State Collaborative (MC). Lisa Dulsky Watkins, MD, the Director of the MC—and three other experts with first-hand experience working with states—were interviewed and identified 10 lessons for achieving comprehensive primary care transformation.
The issue brief looks at topics such as leadership and engagement; public and private support; multi-payer participation; transformation that includes IT and team-based work; evaluation and data; and the future of primary care transformation, including discussion of a new provider category.
