New York City’s attempt to transform traditional welfare medical care arrangements into an integrated and equitable health care system raises cautionary signals for national policy. In the decade following the introduction of Medicare and Medicaid some benefits were realized; but the problems of exploding costs and imploding revenues were beyond local control. The flow of public funds through privately operated systems as yet fails to assure either social purpose or fiscal prudence.
